This application is used to request to join the Average Payment Plan Program. This means that an average bill amount will be calculated by reviewing your last 12 months of billing statements. You will pay the same payment amount each month. The Average Payment Plan payment amount is reevaluated in April and October of each year.
This form should be used to request that your water utility service be reconnected pursuant to Engrossed Substitute House Bill 1329. This new State Legislation allows for residential dwellings to have their water utility service reconnected during heat-related events as determined by the National Weather Service.
This form should be used to request that your water/sewer utility service be turned off for winter OR be turned off temporarily for a repair. Request to Snowbird Off should be for a minimum of 3 months.
